Supported Community Living
Supported Community Living is a specialized community-based program for adults with disabilities who choose to live independently, or who have the goal of independent living in the next few years. It offers one-on-one support to increase or maintain personal self-sufficiency, facilitating an individual's achievement of his or her personal goals. Through SCL programming, North Star addresses the desires, abilities, needs, health, and safety of people living in their own homes or apartments in the community.
North Star’s SCL services utilizes a person-centered approach and provides support and education in:
- Self-Advocacy
- Communication
- Independent Living
- Daily Living Skills and Self-Care
- Money management and budgeting
- Menu-planning, shopping, cooking
- Medication management, follow-through with health providers orders
- Housekeeping
- Home maintenance
- Home and community safety
- Problem-solving and time management
- Social skills
- Scheduling appointments, accessing transportation
- Establishing and using a natural support network.
- Accessing leisure and recreational opportunities
North Star provides CDAC and Respite programs, which are overseen by the SCL program.
CDAC (Consumer-Directed Attendant Care):
Allows eligible persons to obtain in-home assistance with personal care and activities of daily living to increase or maintain independence at home in the community.
Respite Care:
Allows eligible persons to receive the necessary care to give temporary relief to the usual caregiver, allowing someone to remain in their current living situation.
In the persons home, or in the community; typically, within a 30-mile radius of a North Star office location. Offices are in Mason City, New Hampton, Vinton, Waterloo, and Waverly. Counties served include (but are not limited to) Black Hawk, Bremer, Benton, Buchanan, Butler, Cerro Gordo, Chickasaw, Floyd, Grundy, and Tama. Call to inquire if services are provided in your area.
Services are funded through: Medicaid HCBS – BI Waiver, ID Waiver, Home-Based Habilitation Services, County or Region Funding, or Private Pay. Other options are also available for CDAC and Respite Services: Medicaid HCBS – PD Waiver, Elderly Waiver, Health & Disability Waiver, or AIDS/HIV Waiver. Call for other options.
