Supported Community Living |

Who is served:
Adults and youth with a disability who
choose to live in the community or who have a goal of living in the
community. People with disabilities who could benefit from training &
support at home or in the community to help them learn self-care skills,
community integration, and interaction skills. |

What is offered: Person-centered planning directed toward
Supported Community Living. Self-advocacy training. Establishing, finding, and
using natural network of support in the community. Training in independent
living skills: self-care, social skills, home maintenance/ housekeeping, and
community access. |
| When: Day or evening, weekdays or weekends. |
| Where: In the person's home and in
the community: anywhere in Black Hawk, Benton, Bremer, Buchanan, Butler,
Cerro Gordo, Chickasaw, Fayette, Floyd, Grundy, Hardin, Marshall,
Mitchell and other counties (call to inquire about new counties added). |
| How: Funded through: Medicaid Home
and Community Based Waiver (HCBS), which is a mix of federal, state, and
local funds; County's Board of Supervisors; Adult Rehabilitation
Opportunity; Merit Behavioral Health; can also be funded by self/
private pay. |
