| Service Definition |
| Case management/ service coordination services provide goal-oriented
and individualized supports focusing on improved self-sufficiency
through assessment, planning, linkage to other community services,
advocacy, coordination, and monitoring activities. CM/ SC services
are provided in the home and community of the persons served and are
designed to support their twelve counties from offices in Waterloo,
Waverly, New Hampton, Charles City, Marshalltown, Vinton, and Mason
City. Psychosocial rehabilitation services emphasize maximizing good
mental health through treatment and support. Services include
medications monitoring and support, coordination with mental health
professionals, and assistance in accessing community resources. |
Program Goals:
- Persons served maximize their level of independence at home and
in the community.
- Persons served live in healthy and safe environment.
- Persons served participate in activities they enjoy.
- Persons served optimize their mental health treatment, thus
avoiding hospitalization.

Admission Criteria:
- Consumer chooses to participate in the program.
- Consumer has a mental health condition that presents barriers to
independent living in the community.
- Consumer currently lives in or identifies living in own home or
apartment in the community as a future goal.

Service Components:
- Person centered planning.
- Self-advocacy training.
- Establishing, finding, and using natural network of supports in
the community.
- Training in independent living skills.
